This page has been replaced by https://wiki.mozilla.org/CA/Application_Instructions

There is a link in step 2 of the How_to_apply page that will automatically create the bug to request root inclusion, with all of the appropriate fields filled in and with a template in the bug description field.

Using the link, a bug will be created with the following fields set:
* Product: NSS
* Component: CA Certificate Root Program
* Version: Other
* Severity: enhancement
* Platform: All
* OS: All
* Summary: Add [your CA's name] root certificate(s)
* Description: [see below]

Note: It is strongly recommended that you use the link in step 2 of the How_to_apply page to avoid errors in creating the root inclusion request.  However, if needed, you can manually create the bug: Go to the Bug Entry Page, enter "Certificate" in the search bar, and select "CA Certificate Root Program". Then select the fields as listed above.

If you create the bug using the link on the How_to_apply page you will get the following template in the bug description field.

You must also provide all of the information listed in the CA Information Checklist.
