ORGANIZATION NAME

Your organization's name. We will use the organization names to allow people to search for your badges and programs.

PROGRAM NAME

The name of your summer program that the badges will be issued from. We will use the program names to allow people to find your badges associated with this program.

PROGRAM DESCRIPTION

Describe your program as specifically and succinctly as possible. We will use this information on the information page about your summer program and badges.

PARTICIPATION: Estimated number of youth in the program

The expected number of participants per program this summer. And if your program is already fully booked, please indicate that. We will use this information to estimate the number of badges issued, as well as the options for youth looking for additional learning opportunities.

START: Summer program start date

Indicate the program start date. If there are multiple start dates throughout the summer for this program, please clearly indicate them. If your program does not have a start date, write, "Not applicable" or "Runs continuously" whichever is appropriate. We will use this information to map out the learning opportunities across the summer. We will also use this information to help potential badge earners find your program and participate.

END: Summer program end date

Indicate when your program concludes. If there are several start and end dates throughout the summer, please clearly indicate that. If there is no end date, please indicate that by writing, "Not applicable." We will use this information to think through learning pathways and also to consider the frequency with which someone might earn a badge. We will also use this information to help potential badge earners find your program and participate.

LOCATION: Program Address

Indicate the primary location where the program will be occuring. If there are multiple locations, please indicate all locations that will be issuing a badge. If there are multiple locations but they will all be issuing the same badge, please enter those locations individually. We will use this information to allow interested parties to find your program and participate.

CONTACT: Email address

Indicate your organization's primary contact OR the individual program lead's contact information for the summer of learning program. This will be the primary contact for Mozilla and CSOL to contact you should question arise about your badges.

BADGE NAME: Name that badge earners will see

Indicate the name of your badge here. The badge name typically is shown on the front of the badge. Be as descriptive and as succinct as possible. You may wish to include your program name here but it is not a requirement. We will use this information to help people find your badges. This information is also included in the metadata of your badge.

BADGE DESCRIPTION: Short tweetable description of the badge

Write a brief description explaining your badge. Try to keep it to 140 characters, if possible. We will use this information on the badge description page on the CSOL website, and connect you to additional learners. This information is also included in the metadata of your badge.

LEARNING: Describe the learning experience that you want to badge.

Indicate the learning experience or outcome that you are badging.

SKILL: What specific skill or achievement will the badge recognize?

Indicate the specific skill(s) or achievement(s) that this badge represents.

STEAM: What STEAM category (or categories) do you feel this falls under?

Indicate with category or categories - Science, Technology, Engineering, Art, Math - that your badge addresses. Recognize that your badge may cross STEAM boundaries, if so, please indicate those that seem appropriate. We will use this information to develop pathways for participants.

CRITERIA: List out the requirements for the badge

Indicate the criteria that someone needs to do in order to get the badge. Be as specific as possible so that earners can know what is expected of them. This information will feed into the criteria page that is linked to from the badge.

ASSESSMENT: Explain how this skill or achievement will be assessed.

Indicate how a participant's work will be assessed. If possible or relevant, identify Expert, Peer, Self or Embedded/Automatic. This information will feed into the criteria page that is linked to from the badge.

RUBRICS: List any rubrics used for this badge

Indicate if you will be using a rubric when assessing whether or not someone has achieved the work the badge asks the participant to do. Are there particular rubrics that assessors will be using when they review participant's work? This information will feed into the criteria page that is linked to from the badge.

ALIGNMENT: If this badge aligns with any standards, list below

Indicate any and all standards that your badges align with. Be as specific as possible. This information will feed into the criteria page that is linked to from the badge.

PREQUISITES/DEPENDENCIES: Does this badge connect to any of your other badges?

Indicate how your badge relate to other badges that you offer. For example, is this badge required to get another badge or vice versa? If your badge works with other program's badges, then please be as specific as possible about those relationships. We will use this information to develop and highlight learning pathways not only through programs and organizations but also across categories.

TIME: List estimated amount of time it will take someone to earn the badge

Indicate the length of time it will take someone to complete this specific badge. Does it run the length of the program, can it be earned in a day or over the course of an afternoon? Will they have an opportunity to earn this badge multiple times? We will use this information to help people figure out when they can participate and earn badges across the summer.

AUDIENCE: What age groups are most likely to earn this badge? If there is a specific age group, please indicate.

Indicate the age range that you anticipate your audience is. Specifically, which of the following: under 13, 13-18, 19-25, 26+ We will use this information to help people find appropriate projects and programs to participate in.

DELIVERY: Where will someone earn this badge?

Indicate where the badge will be earned. Specifically, which of the following: Offline in face-to-face sessions or events, through email outside of the session meeting times, through our website, other. We will use this data to help people find locations where they can participate and earn badges.
